Dear Sir/ Madam,

RE: IRREVOCABLE AUTHORITY TO DEDUCT FROM MY SALARY

1.  You are hereby authorised and directed to deduct the sum of   K480.00   from my salary every fortnight (including annual leave salaries) and transfer the said sum to the Bank Account of Black Stone Finance Limited (Account Number: 1010223327) at the Branch of the Bank of South Pacific until the total sum of   K7,200.00   has been repaid to Black Stone Finance Limited; and

2.  In the event of my resignation, termination or, if I otherwise leave the employment of National Parliment, the balance of the total sum that remains unpaid may be deducted from any accrued entitlements such as annual leave, long service leave etc. and paid to the account of Black Stone Finance Limited.

The above order is given for valuable consideration, is irrevocable and may not be amended or cancelled without the written consent of Black Stone Finance Limited.
